What to Bring
You can submit patient registration forms online prior to your appointment. Otherwise, we ask that you arrive at your appointment a few minutes early to complete the necessary paperwork. Please bring these items with you:
- Valid form of ID, such as a driver’s license
- Referral slip, if applicable
- Images or X-rays from your general dentist or referring provider
- Insurance information
- List of current medications and/or conditions
- Form of payment
Parents of Minors
Patients under the age of 18 must be accompanied by a parent or legal guardian to the consultation appointment.

What to Expect
During your first visit, we will examine your oral health, talk about your concerns and goals for treatment, and explain all of your options. If you do not have 3D scans already, we will take them at this appointment. These detailed images help our surgeons formulate your treatment plan.
